CareHarmony’s Care Coordinators(LPNs) work comprehensively with providers to deliver value-based care management initiatives for their patients. Our clinicians are driven by their passion for the level of care delivered; experts in assisting patients and caregivers navigate a sometimes fractured healthcare system and consistently prioritize a high-quality standard so patients may better manage chronic illnesses and achieve success.
CareHarmony is seeking an experienced Licensed Practical Nurse(LPN), with at least 3 years of direct patient-facing work experience; that thrives in a fast-paced environment, is self-motivated, has impeccable attention to detail, and values the impact they can patient’s healthcare journey.
You will have experience identifying resources and coordinating needs for chronic care management patients.
You will have a proven ability to patient’s healthcare summary and prioritize.
You will apply your experience and judgment to complex patients.
In this role, you can expect to manage an assigned census of patients; where you will deliver telephonic support, resolve opportunities for care, and coordinate care resources. This position is 100% remote, telephonic, with no face-to-face contact with patients.
Full-Time Monday – Friday, 8 am – 5:30 pm-CST., no weekends
Responsibilities/Day in the Life
- Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
- Identify and coordinate community resources with patients that would benefit their care.
- Provide patient education and health literacy on the management of chronic conditions.
- Perform medication management, including identifying potential medication concerns, reconciliation, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services to your patients; Home Health, DME, Home Infusion, and other critical needs.
- Resolve patients’ questions and create an open dialogue to understand needs.
- Assist/Manage referrals and appointment scheduling.
- Ability to consistently adhere to quality compliance and care-time metrics
Requirements:
- Three years of clinical patient-facing experience; preferably with chronically ill patients.
- Case Management, care coordination, or home health experience preferred.
- Active Illinois LPN License
- Fundamental experience with technology; Microsoft Office Suite or related software
- Excellent written and verbal communication skills; a stickler for details
- Positive, uplifting personality with a compassionate and friendly demeanor
- Natural ability to manage stressful interactions, with a strong concierge approach to potential escalations
- Enjoys working in a fast-growing, rapidly changing environment
- Experience with Epic a plus
- Bilingual skills are a major plus
Remote Requirements:
- Remote work for this position requires you to provide and meet the internet requirements; Fiber or Cable, preferably wired.
- Must have a home office or HIPPA compliant workspace that is secure with privacy to protect personal health information.

Position Details
The Call Center Sales Supervisor, reporting to the Chief Growth Officer, is charged with ensuring that the organization’s strategic plan, mission, vision, and values are communicated to the team and integrated into the team’s initiatives.
In this role, the Supervisor will be responsible for the supervision of 12-20 Patient Enrollment Specialists(PES), an outreach team charged to telephonically educate and enroll patients in a Medicare-sponsored care coordination program designed to help the patients better manage their chronic illnesses (diabetes, high blood pressure, COPD, etc.).
The Supervisor is the point of expertise and accountability for the PES team; planning, directing, and overseeing the outreach operations as well as providing training, coaching, resources, knowledge, and infrastructure needed to meet company enrollment goals.
The ideal Call Center Sales Supervisor has a passion for the coaching and development of his/her team, channeling all team members to overcome challenges, exceed expectations, driving of their success.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Manage, develop, coach, and motivate PES remote team members to develop their skills to ensure that a high professional standard is achieved
- Create coaching/management strategies that yield higher levels of performance to ensure that Key Performance Indicators (KPI) and Service Level Agreements (SLA) are met
- Leading effective and thorough conversations that enhance team performance through coaching, delivery of people management, and performance reviews
- Assess the challenges and successes of each PES team member and apply coaching and development accordingly
- Works with the CGO to execute, administer and accurately manage a variety of analytical data including but not limited to attendance, performance, and quality monitoring
- Drive team performance and setting the standard by contributing to the team’s daily telephonic outreach goals
- Develop and execute training protocols, teaching aids, and performed examples for new and current team members
- Collaborates with the growth and operations teams to manage patient prospecting list
- Strong focus on daily coaching opportunities to drive performance
- Assists with the creation & modification of phone scripts and adherences
- On-site position in Brentwood, TN (REQUIRED)
Requirements
EDUCATION AND EXPERIENCE:
- Bachelor’s Degree with a Business, Marketing, Management, or other related fields preferred
- (exp in lieu of education considered)
- Minimum of three(3) years supervisory, management, or training experience preferably within a call center, customer retention, enrollment environment managing direct reports
- Proven reporting skills identifying KPI trends in a fast-paced environment
- Demonstrated ability to perform essential functions as listed above
- Healthcare experience preferred but not required

- Thorough working knowledge of the concepts of effective supervision of others
- Ability to assign, monitor, and provide feedback in a positive and constructive manner
