Responsibilities

  • Supervise assigned support staff and/or temporary worker on an “as-needed” basis in their duties to support LI-C projects.
  • Manage components / cohorts of clinical trials, including training of participants, data collection, data entry and report preparation within timeframes.
  • Oversee building maintenance, office equipment and supplies. Make recommendations to Assistant Regional Manager for services needed and/or additional equipment.  Communicate with various vendors and contractors and oversee follow through on their work.
  • Plan events including catering, editing and collation of event materials. Coordinate event activities, including audiovisual equipment.
  • Disburse interviewer funds and track subject fee transactions. Monitor and complete accounting reports.  Input, update, and reconcile account information.
  • Minutes – Review and edit meeting minutes/agendas, draft minutes for review and distribute minutes as assigned.
  • Assist with project reports, including reading and editing. Assist team with in compiling, assembling, and editing material for reports.
  • Develop and manage databases for LI-C managed projects. Create Excel worksheets and MS-Access reports.
  • Assist with preparation of IRB materials for submission. Help monitor and enforce model-fidelity in multi-site studies.  Oversee participant recruitment and informed consent process.
  • Manage tracking data – Oversee the maintenance and updating of all tracking information across projects in a variety of formats including training calls, new project file set up, etc.
  • Material preparation – Assist team staff in producing, reproducing, compiling and assembling materials for reports, meetings and trainings, including CDs, manuals, presentations, and reports.
  • Prepare graphs, charts, tables for professional presentations.
  • Documentation – Coordinate, organize and assist with internal documentation updates and maintenance.
  • Project/office file management – Assist principal investigator and project coordinator with organization of project and/or office files.
  • Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.

Qualifications

  • Minimum of a bachelor’s degree in social science or public health with one-year experience working with study population.
  • Intermediate computer skills using Word and Excel required.
  • Excellent organization and communication skills, time management, and attention to detail required.
  • Must have some knowledge of database management.
  • Demonstrated record of on-time follow through with small and large tasks is essential.
  • Demonstrated ability to work independently.
  • Strong service ethic and the ability to communicate clearly and effectively with internal staff at the highest level in the organization and negotiate with contractors and vendors.
  • Must obtain GAIN/ABS certification within 6-months of hire date.

Responsibilities

In consultation with the Assistant Regional Manager, during the follow-up phase the Case Tracker/Interviewer is expected to:

  • Contact clients, collaterals, and other individuals in an effort to schedule and confirm clients for their follow-up appointments.
  • Conduct interviews on and off site.
  • Network to expand available contacts.
  • Participate in regular case review meetings.
  • Meet quality assurance standards as specified by QA Supervisor.
  • Consult with the Research Project Manager and other case trackers on hard to find cases.
  • Document all follow-up activities on the same day of completion.
  • Adjust work schedule as required.
  • Implement duties outline in data collection protocol within specified time frame.
  • Verify new cases within five (5) days of receipt or notify Assistant Regional Manager in writing.
  • Complete minimum of one activity in each assigned open case.
  • Conduct interviews when scheduled.
  • Other duties consistent with prior training experience including, but limited to: answering phones, faxing, data entry, filing and photocopying.
  • Urine collection when assigned.

Responsibilities

  • Complete or coordinate the completion of all required data entry and paperwork, accounting to all funder requirements and in a timely manner, including but not limited to, progress notes, opening paperwork, applications for patient assistance programs, all medical/nursing forms and mental health assessments and treatment plans when assigned:
    • 100% of medical/nursing information to be completed and entered at the time of the delivery of service.
    • 80% of progress notes to be submitted within forty-eight hours of the delivery of service.
  • Review medical records to ensure completeness including all prescriber and consumer signatures are obtained as required.
  • Complete services, including but not limited to medication administration, monitoring and training and case management services as indicated on treatment plans.
  • Assess consumers for acute physical issues, providing first aid as indicated, and review physical health screenings, making referrals or coordinating prescriber reviews when indicated.
  • Act as liaison between prescribers, consumers, laboratories, PCP’s, other healthcare providers, pharmacies and staff to ensure continuity of care.
  • Respond appropriately and timely, to all consumer and family member inquiries regarding medication refills, questions and/or concerns.
  • Administer, evaluate, and appropriately document TB skin tests.
  • Promote and provide medical teaching from a holistic approach per nurse practice guidelines.
  • Promote the recovery model of treatment including hope, respect, empowerment, health/wellness and spirituality/connectedness, using trauma-informed care, through self-directed recovery, including using recovery language in all applicable documentation.
  • Promote positive interactions within Chestnut and the community to sustain a favorable program reputation.
  • Function as a team member, attend meetings and consumer staffings, participate in program development activities and provide support and back-up to other team members as necessary.

Responsibilities

The Targeted Care Specialist conducts street outreach to develop relationships and build rapport with clients. An ideal candidate will use supportive, strengths-based engagement strategies, including motivational interviewing and harm reduction principles. Duties include assisting individuals with accessing appropriate mental and physical health services, housing, substance use treatment, employment and any other services that assist an individual to improve his/her quality of life. The Targeted Care Specialist is the primary point of contact with program participants and provides screening, triage, crisis intervention, and referral to supportive services, with the goal of assisting individuals with moving towards their recovery goals. Candidate must be organized and complete all necessary paperwork to track client progress, and meet with established clients on a weekly basis to review, evaluate, and support recovery goals.

Qualifications

  • Must have a high school diploma or equivalent and five (5) years’ supervised mental health experience;
  • Or a bachelor’s degree in counseling, social work, education, psychology or related human service field;
  • Or a bachelor’s degree in any other field with two (2) years’ supervised clinical experience in a mental health setting;
  • Or a certificate of psychiatric rehabilitation from a DHS-approved program plus a high school diploma or equivalent and two (2) years’ experience in providing mental health services;
  • Or a Certified Recovery Support Specialist (CRSS) in good standing with the State of Illinois.
  • Must have a valid driver’s license, private automobile insurance and be insurable.
  • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).

  • Supervise residential youth AOD clients in the facility during designated hours according to the established schedule by being present on the unit, knowing the location of all residential clients (including during emergency situations) and monitoring their interactions with each other.
  • Maintain a safe living environment for youth residential clients by understanding signs and risks of harm to self and others and following the approved procedures for Non-violent Crisis Intervention, Suicide and Run Alerts and Basic First Aid.
  • Provide specific behavior and emotion management skills to AOD clients, by reinforcing positive coping strategies, encouraging clients’ strengths and redirecting clients to more healthy coping choices.
  • Facilitate AOD groups according to daily schedule including psycho-educational skills groups, self-help group, recreation, and goals or goals wrap up.
  • Assist in completion of specific AOD objectives in individual treatment plan, including conducting individualized A-CRA procedures.
  • Assist in serving meals and snacks according to daily menu/dietary requirements.
  • Monitor and document scheduled and random urine screens for same gender youth AOD clients.
  • Monitor self-administration of medication according to Medication Administration Record (MAR) and ensure all storage, security and documentation policies are followed.
  • Attend and participate in staff meetings, maintaining objectivity and acting as an advocate for the client.
  • Participate in program and staff development activities, including supervision, initial training and competencies, mandatory trainings and achieving ACRA certification and maintenance of fidelity according to policy.
  • Maintain records of clients and services provided including progress notes, group scores, admission and discharge paperwork, daily duty sheet and incident reports.
  • Maintain and protect confidentiality and security of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and client information.

Qualifications

  • Must be 21 years old with a high school diploma or its equivalent.
  • Bachelor’s degree in a related field is preferred, but not required.
  • Must display an interest and ability to work with youth effectively.
  • Must regularly be available to work overnight shifts.
  • Availability to work overnight on weekends a plus.
