  • Primary Responsibilities: 
    • Initiate, monitor, and discontinue hemodialysis treatment for assigned patients, according to PSKC procedures.

    • Provide thorough and timely routine assessment of internal access and central venous catheters including skilled cannulation and access for dialysis treatments.
    • Conduct regular assessments and provide thorough documentation per policy and procedure.
    • Prepare and administer oral, subcutaneous, intramuscular, and intravenous medications per protocol or physician order. 
    • Order labs, review laboratory data, and apply protocols. 
    • Maintain daily clinical records with accurate and timely documentation in the Electronic Medical Record.
    • Implement current patient treatment care plan.
    • Maintain accurate and current patient medication lists on assigned patients.
    • Appropriately identify, communicate, and intervene with regard to medical crisis or complications.
    • Directly identify and address patients and/or guests who demonstrate challenging and disruptive behaviors. 
    • Intervene in situations of conflict involving patients, staff and/or guests.
    • Under the direction of the registered nurse, implement interventions to meet primary patient’s dialysis needs.
    • Attend staff meetings when scheduled.

    • Assist in transferring patients between wheelchairs, dialysis chairs, hospital beds, stretchers, etc. 
    • Use transfer equipment such as sliding boards, transfer belts and Hoyer lifts per policy and procedure.

    • Assist patients in transferring onto and off of the toilet.
    • Assist patients with personal hygiene care (peri-care) as needed.
    • Assist patients in the use of bed pans.
    • Maintain effective communication with the patient and the Interdisciplinary team (IDT) and refer patient needs to members of the (IDT) as necessary.
    • Provide leadership and appropriate supervision to Dialysis Technicians.
    • Participate in rotating float or on-call schedule as needed.

Maintain patient locker lists and keep updated.
Maintain staff locker lists and keep updated (task assignment is dependent on location).
Maintain and update Care Facility List.
Maintain and update Home Patient list (task assignment is dependent on location).
Monitor office supply inventory and order as needed.
Run reports and distribute as requested.
Sign in new or visiting patients.
Process Admission Record changes and distribute to staff.
Distribute educational and communication materials to patients and other staff as needed.
Distribute Standing Orders accordingly.
Maintain inventory of all medical forms.
Fax nursing home information as instructed.
Monitor fax machine and promptly distribute faxes to staff mail boxes.
Fill copy and fax machines with paper.
Routinely check unit administrative inboxes (frequency is based on location).
Run emergency reports and maintain unit emergency backup notebook.
Distribute access list to nurses at end of each month.
Scan and upload medical records to the Electronic Medical Record.
Coordinate meeting room schedule.
If other staff not available (social workers, etc.), assist patients with contacting transportation companies.
Update the administrative communications file on new and transfer in patients.
Perform lab processing tasks, including entry of lab orders into the lab system, printing labels and collection lists, labeling blood tubes and making lab draw kits for nursing staff.
Maintain the email group distribution list for the assigned location(s)/department(s).
Make arrangements for traveling patients (outgoing) and transfer out patients.
