Full Job Description

The Vanderbilt Home Care Social Worker 2, travels to the patients residence to provide psychosocial assessment of and brief targeted intervention for social, emotional and environmental issues that impact client/family’s ability to optimally benefit from care. Actively collaborates with team to meet patient care management and system improvement goals. Represents the Social Work department on institutional and community committees/task forces. Provides leadership for department committees/workgroups. Contributes to the plan of care with knowledge of population specific needs as it relates to the identification of patient psychosocial needs.

Position Description:

The Social Worker 2 for Vanderbilt Home Care Services will be serving the home health population, doing 3 to 4 minimum home visits daily.  Working closely with other home care clinical staff (Nurses, PT, OT, and SP).  Must have Master’s Degree and be a Licensed Master Social Worker.

Sign On Bonus:  $5000 (paid out after 60 days of employment) (Prior employees must be separated from Vanderbilt for at least 1 full year to qualify for the bonus)

JOB SUMMARY

The LifeFlight Ground Transport Coordinator (Dispatcher) is an integral part of a comprehensive community-based EMS system, designed to provide both high-acuity urgent interfacility transports to a higher-level care and scheduled non-urgent transport of patients from VUMC hospitals to post-acute care facilities or home.  LifeFlight Ground EMS also responds to requests for mutual aid to support any community 911 or hospital system during times of natural disaster, mass casualty, or other extreme circumstances.  Each LifeFlight Ground transport base is specifically aligned in communities requiring direct support of VUMC hospitals or partnering facilities; however, the entire program responds to requests across a multi-state region.

The LifeFlight Ground Ambulance Transport Coordinator is responsible for planning, coordination, and dispatch of all emergent and non-emergent LifeFlight Ground Ambulance transport requests as well third-party transport vendors who augment transport requests to and from VUMC facilities. The Transport Coordinator must be capable and comfortable providing exceptional quality and service to a wide variety of internal and external customers to quickly triage each request and dispatch the most appropriate resource.  The transport coordinator must maintain constant awareness of each LifeFlight Ground Transport resource stationed and deployed across a multi-state region to maximize the safety, efficiency and effectiveness of our teams and the patients we serve.  Provides information and emergency notification to customers.  Works with advanced communications systems.  Practices continuous professional development and excellent customer service.  Practices proactive operational support of the department.

POSITION SHIFT

  • Full Time, hourly (Non-Exempt): 12-hour shifts are primarily utilized to fulfill this role, however, there are some variable shift hours possible during peak hours of service.

SIGNING BONUS

  • $3,500.00 signing bonus for Full-time EMTs and $5,000.00 for Full Time Paramedics fulfilling this role, first half paid after six months of employment and second half paid after 12 months of employment.

Job Summary:

The paramedic is responsible for assessment, treatment, and transport of all types of patients and conditions, and must be able to provide the level of care required to match the patient’s needs (ALS, BLS or Specialty Care).  The paramedic must initiate and or adapt patient care plans according to scope of practice requirements and LifeFlight approved treatment protocols and medical direction.  The paramedic is responsible for continuously reassessing and adjusting care as conditions change with the goal of always optimizing outcomes. The paramedic is expected to always deliver exceptional compassion, professionalism, patient care, and customer service.  Effective communications and close collaboration with EMS team members, hospital staff, communicators, and other key stake holders is essential.

The paramedic will communicate with all medical personnel and facilities’ staff in a respectful manner while obtaining instructions or feedback for transport and hand-off.  The paramedic will communicate clearly and concisely via radio to relay critical and time sensitive patient information to receiving facilities.  The paramedic will complete all required documentation appropriately and in a timely manner.  The paramedic will always operate an emergency vehicle and equipment in a safe and effective manner. The paramedic will work closely with their teammates to check, restock, inventory and clean emergency vehicles and supplies as to stay ready for a timely response.  The paramedic will stay in good standing with state and institutional requirements.

Bachelor’s degree (or equivalent experience) from accredited institution and 1 year relevant experience. Medical Laboratory Scientist Certification or Medical Laboratory Technologist Tennessee license. If not licensed, must be eligible to obtain license in 120 days.

License as a Medical Laboratory Technologist generalist or categorical, or

  • Successful completion of CAHEA or NAACLS accredited medical laboratory science program or BS degree from an accredited institution with major in a biological, chemical, physical, or clinical laboratory science, or
  • Equivalent education and experience per CLIA and the Tennessee Medical Laboratory Board (1200-067-01-.22) for personnel performing high complexity testing, AND
  • ASCP medical laboratory scientist generalist or categorical certification, or equivalent certification.
